All Hazards Mitigation Plan
This plan identifies the risks to West Virginia from natural hazards like flooding, winter storms and fire; quantifies their costs to reveal which hazards have the greatest impacts; coordinates various stakeholders in all levels of government, private enterprise, nonprofits, educational institutions, and individual citizens to examine the costs of disasters and their vulnerability to disasters; empowers stakeholders to identify and prioritize actions to mitigate the effects of disasters; and creates ways for the responsible stakeholders to implement these actions, monitor their progress and periodically reassess priorities. It serves as an important step in an effort to improve the quality of life for present and future generations of West Virginians.

Bioterrorism Clinical Specimen Submission Form
Authorized persons must submit this form only for samples submitted to the Threat-Preparedness & Bioterrorism Response Section (BT Lab) for identifying potential Bioterrorism Agents. Verbal authorization must be received from the BT Lab prior to sending any specimens.
Bioterrorism Environmental Specimen Submission Form
Authorized persons must use this form only for Environmental samples submitted to the BT Lab for identifying potential Bioterrorism Agents. A Chain of Custody document for each sample must also be submitted.

Standard State Mitigation Plan
The mitigation plan demonstrates the State’s commitment to reducing the risks from natural hazards, and should serve as a guide for all levels of State decision makers. The plan should detail how the State will address planning for natural hazards and the resources they are going to commit to the process.
