The mission of the West Virginia Division of Health Promotion and Chronic Disease (HPCD) is to advocate for chronic disease management and prevention. Chronic diseases are illnesses or conditions such as cardiovascular disease, asthma, or arthritis that last for a long period of time and may require ongoing medical care and supervision.
